How can I compare Medicare options without overlooking access, drug costs or enrollment rights?
Compare the doctors, medicines and services you need, then the total cost and enrollment rules. A low plan premium is only one part of the decision. Confirm Medigap purchase rights before relying on a plan to leave Medicare Advantage.

What this guide helps you do
This guide helps people with Medicare and their caregivers organize a coverage comparison in the United States. Local plans and costs depend on geography and coverage year. Employer, retiree, Medicaid, VA or other coverage may change the decision; confirm coordination before changing existing coverage.
Keep the coverage routes separate
Original Medicare is Part A and Part B. A person may also need a separate Part D drug plan and may consider Medigap, which is private supplemental insurance. Medicare Advantage is a Medicare-approved private-plan route to Medicare benefits; most plans include Part D. The official comparison explains the main differences.
System comparison; read the actual plan documents before choosing
| Question | Original Medicare | Medicare Advantage |
|---|---|---|
| Which doctors? | Generally any U.S. doctor or hospital that takes Medicare; confirm acceptance and assignment. | Non-emergency care may require a plan network and service area; out-of-network terms vary. |
| Referrals and prior approval? | In most cases no specialist referral or prior authorization for covered care. | Referrals or prior authorization may apply to particular services. |
| Premiums? | Part B premium; separate drug and supplemental premiums may apply. | Part B premium remains; the plan may have another premium, including a possible $0 plan premium. |
| Annual cost exposure? | No yearly limit without supplemental coverage or other insurance. | Annual limit for covered Medicare services; verify in-network/out-of-network terms and separate drug costs. |
| Supplement and drugs? | Separate Part D and Medigap may be relevant, subject to eligibility and timing. | Most plans include Part D. Medigap cannot pay this plan’s cost sharing. |
This table summarizes CMS guidance. It does not compare individual premiums, dental limits, medicine formularies or local provider contracts. Benefits that sound similar can have different conditions.
Check the calendar before changing coverage
The annual fall Open Enrollment page states October 15–December 7, with changes effective January 1 of the next year; the plan must receive the request by December 7. For fall 2026 choices, use the 2027 coverage year for a January 2027 change. Other enrollment routes depend on circumstances.
Medigap has a separate timing question. The usual federal Medigap open-enrollment period is six months beginning with the first month you are 65 or older and enrolled in Part B. Outside it, purchase rights depend on circumstances and state law. The Medigap navigator shows several conditional paths. A general right to switch coverage during fall enrollment does not itself establish a right to buy the supplement you want.
Make a comparison you can use
- Collect the starting facts. Gather current coverage cards, the Annual Notice of Change if applicable, regular doctors and hospitals, medicines with exact doses, pharmacies and expected care needs. Keep sensitive data on the official service or with a trusted counselor.
- Choose the correct year and location. The Plan Compare landing page offered 2026 and 2027 during this review. Enter the ZIP code and relevant county when asked. Do not compare next-year coverage with an old-year plan result.
- Check medicine costs carefully. Enter the actual medicines and pharmacies. Review the plan’s drug list, coverage restrictions and estimated annual cost. A low premium may not produce the lowest total cost for your medicines.
- Confirm provider access. Ask the plan and the provider about the specific plan and coverage year. Record the date and answer. A provider accepting Medicare is a different question from participating in a particular Medicare Advantage network.
- Compare an ordinary year and a difficult year. List premiums, expected visits, medicines and other routine costs. Then examine the plan’s limits and rules for covered services. Do not add the maximum to ordinary spending and call that a personalized prediction. Identify what is excluded from any limit.
- Check the exit and supplement questions. Before planning to leave Medicare Advantage, confirm the effective dates, any separate drug plan, and whether you can obtain the desired Medigap policy. Ask for individualized help when purchase rights are uncertain.
- Use independent assistance. Colorado SHIP describes free, unbiased and individualized Medicare information. Its page lists 888-696-7213 and county assistance links. Bring the same facts to every comparison. The page also has older documents; check a document’s year before using it.
- Make and retain the decision record. Save the chosen plan ID, coverage year, supporting documents and any enrollment confirmation. Confirm when coverage starts before depending on it. This guide and its review did not enroll anyone.
Decision map: three checks before a coverage change
Right plan, year and location?
Use the correct coverage year and ZIP/county; keep the actual plan identifier.
Needed care and total cost fit?
Check doctors, medicines, approvals, premiums and covered-service limits.
Enrollment and supplement rights?
Confirm timing and any Medigap right before relying on a switch.
Text alternative: Complete all three checks: identify the right local plan and year; verify access and costs; then confirm enrollment timing and any separate supplement purchase rights.
A comparison worksheet
- Plan and year: ______. Doctors/hospitals confirmed on date: ______.
- Medicines, doses and pharmacies checked: ______. Estimated drug and health costs: ______.
- Premiums and covered-service limits: ______. Important exclusions or approvals: ______.
- Enrollment route and receipt deadline: ______. Proposed start date: ______.
- Medigap question, answer source and remaining uncertainty: ______.
Avoid these common errors
- Treating a $0 plan premium as $0 health care.
- Assuming a provider participates in every plan from the same insurer.
- Using the wrong year’s medicine prices or network listing.
- Assuming fall enrollment guarantees access to any Medigap policy.
- Canceling or changing coverage before confirming coordination and effective dates.
Your next useful action
Prepare the medicine and provider list, select the coverage year, and compare two realistic routes using the same facts. If Medigap timing or other coverage is involved, resolve that question with SHIP or Medicare before acting.
